No such letter is required!!
submit a photocopy
i had submitted a photocopy of the following the following:
1. all four year marksheets
2. final year completion certificate
3. internship completion cert
4.degree certificate
5.transcripts issued from the college, of the 4 years of bds (sealed and signed)
6.a documented proof of any change in your name or surname in case you have after your marriage.
7.liscensing certificate issued by dci
8. a print out of the completed evaluation request form
9. a cheque of the amount
when you request an evaluation, ask for a 'course-by-course' evaluation. now for taking the nbde part1, they are fine with a general evaluation but when you apply to colleges, they demand a course by course evaluation. so if you do a course by course eval now, later on you ll need to just ask for its copies. but if you do a general eval now, you ll need to ask for a course by course eval later. once again, you can send either a course by course eval or a general eval to the joint commission for taking a nbde date.
with your eval fee, you r entitled to 2 free copies of which 1 mandatorily comes to your residential address. dont forget to enter the address of the joint commission of dental exams , so that they directly recieve a copy of your evaluation from ece itself.
now regarding the total number of copies to order, i had asked for 10, but i feel 5-6 is a reasonable number as sometimes certain colleges require you to mail your evaluation report via ece, as in they need ece to mail it directly to them on your behalf. but every time you need to reorder copies, they charge $30 excluding the price per copy, so taking that into account i feel that 5-6 is a decent no. i ordered 10 coz i was unsure about the programmes i want to apply for.

Re: Documents Required
anyways, when you send the marks sheet n other docs then it'll be obvious that they'll be on the university's letter head...so i dun think you need any letter separately...
